Doctor Barrows is the leader of the Underworld and proprietor of its clinic in Fallout 3.
Doctor Barrows is the informal mayor of the Underworld and proprietor of the Chop Shop and one of the few doctors in the wasteland trying to gain a scientific understanding of the condition of ghouldom. Whenever he is not tending to his patients, he runs experiments on samples gathered from volunteers (skin, hair, occasionally organs) to determine what ghoulification is and whether it can be rectified. He enjoys dark humor greatly, one of the few ways to maintain composure in a taxing line of work.
Recently, some ghouls scavengers brought a badly mangled Reilly to his shop, after finding her near the entrance to the city. Heavily wounded in a skirmish with supermutants (multiple lacerations, contusions, broken leg, some internal bleeding and blunt force trauma), she has remained in a coma, with Barrows unwilling to wake her up, first to allow her body to heal and second because of the potential for damage to her brain.
Barrows was one of several medical specialists contacted by the Railroad to conduct facial surgery on an escaped android. He dismissed it as a hoax, owing to his experience with appearances and gullibility. The fact that he considers medicine to be an important art and wasting it on cosmetic modifications is improper certainly didn't help.
While he tends to crack jokes, he is devoted to his art and will heal anyone who requires attention., even if his patient was a super mutant. Of course, he isn't a charity. He will require payment to cover his expenses.

The word "Barrow" means burial mound, keeping in line with the morbid theme of Underworld.
Doctor Barrows appears only in Fallout 3.

The Lone Wanderer: "What do you mean "fresh human samples"?"
Barrows: "You haven't heard? I'm the foremost authority on Ghoul evolution. I want to know what makes us tick. Something doomed us to this rotting form, and I aim to discover what it is. So, I need samples of human skin, organs and other parts to make the experiments valid."
The Lone Wanderer: "Sorry, I aim to keep all my samples to myself."
Barrows: "Pity. Well then, what can I do for you?"

Doctor Barrows runs Underworld's Chop Shop and has a pretty sick sense of humor. He hangs around the Chop Shop most of the time, often looking in on Reilly, the human female he's treating. At night, he leaves to eat dinner at Carol's Place for a few hours, and then returns to the clinic. He sleeps for a couple of hours on site. Doctor Barrows is also mayor of Underworld, although, because of the small nature of the town, his civic duties take a back seat."

Barrows: "My dear friend, yelling "zombie" in Underworld would be like yelling "sand" in a desert. Pointless. Obviously, you take no appreciation in what I am trying to achieve. Please, leave me be. I have much to do."

Barrows: "That's just the attitude I've come to expect from most people. It's sadly what sets us apart. I'm not going to dissect you alive or tear out your entrails like in some lurid horror movie. All of my experiments are done by the book and scientifically."

The Lone Wanderer: "Reilly's Rangers?"
Barrows: "Oh, you must be new around here. They're a mercenary company that bases itself out of the D.C. Ruins. I'm assuming she's their leader, Reilly. What was left of her armor had her name stenciled on it. She was in a pretty bad way when they found her outside Underworld."
The Lone Wanderer: "What's wrong with her?"
Barrows: "She had multiple lacerations, contusions, broken leg, some internal bleeding and blunt force trauma. She's hasn't been conscious since they brought her in a few days ago. I'd say she tangled with some Super Mutants and lost. It's a miracle she isn't dead."
The Lone Wanderer: "How did she end up in the Chop Shop?"
Barrows: "She was just brought in by a few others. They found her bleeding to death at the entrance to the city here. If she'd been out in the tunnels any longer, she'd have been someone's meal by now."
The Lone Wanderer: "Do you think there's any way to revive her?"
Barrows: "Well, I wouldn't recommend it. She's in some sort of coma. It's dangerous to force someone out of one. There's no telling when she'll come out of it I'm afraid."
The Lone Wanderer: "Do you mind if I try and revive her?"
Barrows: "Unless you're an expert at medicine, I wouldn't recommend it. You may end up doing more harm than good. However, I'm all for helping my patients, so if you think you have the ability, be my guest."

Barrows: "I've learned a thing or two about not taking things on appearances. That was an elaborate hoax. Take this holotape and give a listen. Clearly fake."

Barrows: "It was clearly faked. You can ask practically any doctor in the Wasteland. We all got a tape like this."
- ↑ The Lone Wanderer: "Ever do any facial surgery? Maybe on someone from the Commonwealth?"
The Lone Wanderer: "You ever do a face job on an android from the North? <Jingle purse of 100 caps.>"
The Lone Wanderer: "A patient of mine needs a facial epidermal augmentation. You ever perform one?"
Barrows: "Medicine is an important art and science. Shouldn't be wasted on vanity. Hey, but that reminds me of this old hoax. Here, listen to this holotape."

Barrows: "Don't think because I'm a Ghoul, I'm stupid. You're fine."
Barrows: "Yeah, you look almost as bad as I do. Of course, I'm supposed to look this way and you aren't. 100 caps will do it for my expenses."
Barrows: "Did you get the number of that Brahmin that trampled you? Sheesh! If you want me to stitch you back together, it'll be 100 caps."
