In contrast to previous games, perks exist in the form of "perk cards" which can be traded, upgraded, and reallocated. Card packs are received every two levels until level ten (i.e. at level 2, 4, 6, 8, and 10), after which they are granted every five (15, 20, and so on and so forth). Packs contain cards and a consumable chewing gum. The wrapper of the gum also contains a random joke, written by Emil Pagliarulo. Spare cards of the same type can be combined to create one card of a higher rank.

The number of cards and total card strengths that can be equipped are dependent on the player's relevant SPECIAL stat: The sum total of perk levels cannot exceed the governing perk stat (eg. at Strength 8, the player can use two level 3 perks and one level 2 perk). SPECIAL stats cap out at 15, which means builds will have a 15 point limit per maxed stat. Perks can be swapped out on the go, allowing players to easily experiment with perks.

The Charisma stat governs perk sharing with other players, allowing higher rank perks to be shared.
